Form4506-T

How to fill out Form 4506-T

Form 4506-T asks the IRS for a transcript of your past tax returns, tax account or wage and income records, or a letter saying you did not file. The IRS mails the transcript to your address on record, not to anyone else. Many people can get the same transcripts online instead.

  • Revision covered: Rev. 4-2025
  • Sent to the IRS
  • Checked against irs.gov on 2026-10-23

Form 4506-T on irs.gov Official PDF (irs.gov)

Not tax or legal advice. This guide is our reading of the IRS instructions for Form 4506-T (Rev. 4-2025). It is not tax or legal advice, and no tax professional has reviewed it. Check the official instructions on irs.gov, or ask a tax professional, before you rely on it.

Who needs this form

Use Form 4506-T when you need a record of a past return and cannot or do not want to get it online. Common reasons: a loan or financial aid application, an immigration case, or rebuilding your own records. It covers:

  • A return transcript: most line items of the return as you filed it.
  • An account transcript: payments, penalties and changes made after the return was filed.
  • A record of account: both of the above together.
  • A verification of nonfiling: proof the IRS has no record of a return for a year.
  • A wage and income transcript: data from Forms W-2, 1099, 1098 and 5498 sent to the IRS.

Try the faster routes first. Get Transcript on IRS.gov lets you view, print or download all of these, or you can call the IRS at 800-908-9946.

Pick another form for these cases:

  • A lender or other company that needs your transcript sends you Form 4506-C. Since July 2019 the IRS no longer mails transcripts to third parties, so a 4506-T will not reach them.
  • For a full copy of a return with all attachments, including the W-2s you filed with it, use Form 4506.
  • To let a person you name see your tax information, use Form 8821.

What to have ready

  • The name and SSN or ITIN exactly as on the return, and your spouse's if it was a joint return. For a business, its name and EIN.
  • Your current address, and the address on your last return if it was different.
  • The tax form number (such as 1040, 1065 or 1120) and the years or periods you need.
  • For a business, trust or estate, the document that shows you can sign for it.

Step by step

  1. Enter the name and number on the return Line 1a and Line 1b

Write the name shown on the return; for a joint return, the name shown first. On Line 1b, enter the first SSN or ITIN on the return, or the EIN if you are asking about a business return. A sole proprietor asking for Form 1040 with Schedule C enters the SSN.

  2. Add your spouse for a joint return Line 2a and Line 2b

Enter your spouse's name and the second SSN or ITIN exactly as on the joint return. Leave both blank otherwise.

  3. Give your addresses Line 3 and Line 4

Line 3 is your current name and address, with any apartment or suite number, or your P.O. box if you use one. Line 4 is the address on the last return you filed, only if it differs. If the two differ and you have not told the IRS you moved, file Form 8822 as well, because the transcript goes to the address the IRS has on record.

  4. Leave line 5 empty unless you want a reference Line 5

The customer file number is optional. You may enter up to 10 digits of your own choosing, and they print on the transcript. Never use an SSN or a name: the IRS then prints 9999999999 instead.

  5. Pick the transcript Line 6

Enter one tax form number only, then check one box: Line 6a return transcript, Line 6b account transcript or Line 6c record of account. Return transcripts and the record of account cover the current year and the three prior years. If you are not sure which one you need, the IRS suggests the record of account, because it has the most detail.

  6. Ask for other records if needed Line 7 and Line 8

Check Line 7 for a verification of nonfiling; for the current year it is available only after June 15. Check Line 8 for a transcript of W-2, 1099, 1098 or 5498 data, which the IRS may have for up to 10 years. It does not include state or local information.

  7. Enter the years Line 9

Write the end date of each tax year or period in mm/dd/yyyy format, for example 12/31/2024 for a calendar year 2024 Form 1040. For quarterly returns, enter each quarter.

  8. Check the box and sign

Fill in every line that applies first: the IRS rejects an incomplete form. Check the box that says you have the authority to sign, or the IRS returns the form unprocessed. Sign as your name appeared on the return (and your current name too, if it changed), add the date and a phone number. For a joint return, one spouse's signature is enough. An officer signs for a corporation, a partner for a partnership, and a representative only if Form 2848 line 5 gives that authority, with the 2848 attached.

Where it goes

Mail or fax the form to the IRS, using the state you lived in (or your business was in) when that return was filed. Most requests are processed within 10 business days. If you ask for more than one transcript and the chart shows two addresses, use the address for your most recent return.
