Form4506-C

How to fill out Form 4506-C

Form 4506-C lets a lender or other company get your tax transcripts from the IRS through its Income Verification Express Service (IVES). The transcripts go to that company, not to you. You fill it in, sign it and give it back to the company, which sends it to the IRS.

  • Revision covered: Rev. 10-2022
  • Given to the lender or company that asked for it, which sends it to the IRS
  • Checked against irs.gov on 2026-10-23

Form 4506-C on irs.gov Official PDF (irs.gov)

Not tax or legal advice. This guide is our reading of the IRS instructions for Form 4506-C (Rev. 10-2022). It is not tax or legal advice, and no tax professional has reviewed it. Check the official instructions on irs.gov, or ask a tax professional, before you rely on it.

Who needs this form

You fill out Form 4506-C when a mortgage lender, bank, credit union or other company asks to check your income with the IRS. The company must be an IVES participant: it is registered with the IRS to receive transcripts with your consent. The IRS sends the transcripts to that company only.

This is not the form for getting your own records:

  • To see or download your own transcript, use Get Transcript on IRS.gov.
  • To have a transcript mailed to you, use Form 4506-T. Since July 2019 the IRS mails those only to your address of record, never to a third party.
  • To let a person you name see your tax information, use Form 8821. To let someone act for you with the IRS, use Form 2848.

Usually the company hands you the form with line 5 already filled. If it gives you a blank one, ask for its details first: the IRS rejects the form when line 5 is empty.

What to have ready

  • Your name and SSN or ITIN exactly as on your tax return, or the business name and EIN.
  • Your current address, and the address on your last return if it was different.
  • For a joint return where both spouses are listed, your spouse's name and SSN or ITIN.
  • The company's IVES participant name, ID number, SOR mailbox ID and address, and its client company details for line 5. The company gives you these.
  • Which transcript the company wants, the tax form number (such as 1040) and the years.

Step by step

  1. Enter your name and number Line 1a and Line 1b

Write your first name, middle initial and last name as on your return. For a business, put its name on the last name line. On Line 1b, enter your SSN or ITIN with the dashes, or the business EIN.

  2. Add a previous name if you have one Line 1c

Fill it in only if the name on your last filed return differs from line 1a. Otherwise leave it empty.

  3. Add your spouse for a joint return Line 2a to Line 2c

Fill these in only if you filed jointly and the company wants transcripts for both of you. Your spouse then signs too.

  4. Give your addresses Line 3 and Line 4

Line 3 is your current address, including a P.O. box if you use one. Fill in Line 4 only if your last filed return showed a different address. If you moved and never told the IRS, also file Form 8822 (Form 8822-B for a business) with this one.

  5. Check what the company filled in Line 5a to Line 5d

Line 5a names the IVES participant that receives your transcripts, with its ID number and SOR mailbox ID. Line 5b and Line 5c are optional reference numbers the company may use; they must never contain an SSN, ITIN or EIN. Line 5d is the client company, usually your lender. It cannot be blank or "NA": if the IVES participant is also the lender, its details go on both lines.

  6. Pick the transcript Line 6

Enter one tax form number, such as 1040, 1065 or 1120, and check one box:

    • Return transcript: most lines of the return as filed, without later changes. Available for the current year and the 3 prior processing years.
    • Account transcript: payments, penalties and adjustments made after the return was filed.
    • Record of account: the two combined. Available for the current year and the 3 prior years.

If you don't know which one, ask the company. Leave line 6 empty if it wants only a wage and income transcript.

  7. Ask for wage and income records if needed Line 7

Check it only if the company wants the data from your W-2, 1099, 1098 or 5498 forms. Enter up to three form numbers, or none to get them all. Mark line 1a, line 2a or both; with no box checked, the transcripts cover everyone listed.

  8. Enter the years Line 8

Write the ending date of each tax year or period in mm dd yyyy format, such as 12 31 2025 for a calendar year. The form has room for four. A fiscal year or a quarter uses its own ending date.

  9. Check the box and sign

Check the box that says you read the statement and have authority to sign. The IRS does not process the form if it is unchecked. Then sign, date and add the phone number. If both spouses are listed on lines 1a and 2a, both must sign. A corporate officer, partner or trustee signs with a title, and other entities attach proof of authority. A representative may sign only if Form 2848, line 5a, gives that authority, with that Form 2848 attached and the Authorized Representative box checked.

Where it goes

You don't send Form 4506-C to the IRS yourself. Give the signed form back to the company that asked for it. The IVES participant faxes it to its assigned IVES team with the approved cover sheet, and the IRS sends the transcripts to that company.

Keep a copy for your records. The law bars the company from using or passing on your tax information beyond what you consented to.

Fill it in PDF Sign & Fill

  1. Tap Forms in the bottom bar. In Blank forms, search for 4506-C and open Form 4506-C. It is downloaded from irs.gov when you open it.
  2. Tap Answer step by step. The app asks one question at a time; Next and Back move between them. If an answer shows this is the wrong form, you see This form isn't the one and a button to open the right one. Prefer the page itself? Tap Fill on the page instead.
  3. On Review your answers, tap a line to change it, then Continue.
  4. On Sign the form, pick Use this signature or Draw a new one. If the form must be signed in ink, tap Skip, I'll sign it on paper.
  5. On Check the whole form, read every page, tick I've read the whole form and it is correct and tap Finish and save.
  6. Your signed copy is ready: tap Share to send it, or Save as… to keep it in a folder.

Sign in the app only if the company told you it accepts electronic signatures; otherwise tap Skip, I'll sign it on paper, print it and sign it by hand. Give the signed form back to the company that asked for it: the IRS must receive it within 120 days of the date you sign.

Your first form step by step is free; after that, answering step by step is part of Pro, a one-time purchase. Filling the form on the page, signing, saving and sharing stay free. Your documents are never uploaded.
